The Backflow Test Result (Form 9) Book comes with 25 forms, all in triplicate. These books are designed with hard covers to limit possible damage to the forms contained within.

• Description

  The Backflow Test Result Form Book comes with 25 forms, all in triplicate. The Backflow Test Result form has been designed by MPAQ in accordance with the Form 9 – Registration and report on inspection and testing of testable backflow prevention devices. These books are designed with hard covers to try and limit the possible damage to the forms contained within.

  These forms are to be used to record the particulars of a backflow prevention device used for testing, the results of testing, and whether the device complies with AS/NZS 2845.3.

  This form must be submitted to local government within 10 business days after inspecting or testing the device.

  MPAQ Contracting Members receive a discounted price. Those in associate categories and non-members are subject to full product prices.
  

• Postage and Handling

  If you require postage and handling, there is a flat fee of $15.00. If you would like to pick up your purchase from the MPAQ office, please contact MPAQ on 07 3273 0800 to waive the postage and handling fee.

  Items can be picked up from the MPAQ office between 8:00am-5:00pm, Monday to Friday (excluding public holidays and a two week office shutdown over Christmas and New Year).
